JOB DESCRIPTION – MRD

1. Case sheet Management:

Label case sheets with unique identifiers (e.g., 1, 2, 3) and assign IP numbers.

Perform ICD coding for each case sheet based on the diagnosis.

Scan and digitally store case sheets in the hospital's system for easy retrieval.

Conduct Active Audits and Passive Audits

Active Audit: Verify each document in the case sheet systematically.

Passive Audit: Review archived case sheets for accuracy and compliance.

2. Document Processing and Record Maintenance:

Scanning Register: Record all scanned case sheets.

Bed Occupancy Rate Register: Track daily bed occupancy statistics.

Notifiable Disease Register: Record cases of reportable diseases.

MRD Handover Register: Track the handover of medical records.

Case sheet Issue Register: Record details of issued case sheets.

External Issue Register: Track any external requests for case sheets.

Visitor's Register: Log visitor details in the MRD.

MLC Register: Record medico-legal cases handled by the department.

3. Monthly Data Management:

Compile and maintain monthly data, including:

Monthly Indicators: Performance metrics for the department.

Monthly Statements: Summary of key data points for hospital reporting.

Notifiable Diseases: Monthly data on reportable conditions.

4. File Management:

Maintain organized files for:

Notifiable Disease: Records of reportable diseases.

Indicator Files: Data on performance and quality indicators.

Requisition Files: Requests for medical records.

Abbreviation Files: Standardized list of medical abbreviations used.
