Caregiver – Job Description & Responsibilities

Position

Caregiver / Patient Care Assistant

Job Purpose

The Caregiver is responsible for providing safe, compassionate, and respectful support to patients at home. The caregiver should assist with daily activities, personal care, mobility, basic patient support, and maintaining a clean and comfortable environment while following the instructions of the patient’s family and healthcare professionals.

Key Responsibilities

1. Patient Personal Care

- Assist with bathing, grooming, dressing, and personal hygiene.

- Assist with toileting and changing adult diapers when required.

- Maintain proper hygiene and dignity of the patient.

- Assist with feeding and drinking as instructed.

2. Mobility & Positioning

- Assist patients with walking, sitting, standing, and transferring from bed to chair.

- Reposition bedridden patients regularly to support comfort and prevent pressure injuries.

- Assist with wheelchair and walker use where required.

- Follow safe lifting and transfer practices.

3. Bedridden & Dependent Patient Care

- Provide attentive care to bedridden and dependent patients.

- Maintain proper positioning and comfort.

- Assist with routine personal care and hygiene.

- Observe the patient's condition and promptly report any unusual changes.

4. Basic Patient Support

- Check and report basic observations when trained and instructed.

- Assist with medication reminders strictly according to the prescribed schedule and family/medical instructions.

- Support routine exercises or physiotherapy activities only as instructed by the healthcare professional.

- Assist with catheter, feeding tube, wound-care, or other medical-care routines only when specifically trained and authorized.

5. Nutrition & Hydration

- Assist the patient with meals according to the prescribed diet.

- Encourage adequate hydration when permitted.

- Maintain cleanliness during feeding.

- Report poor appetite, difficulty swallowing, vomiting, or other concerns to the family/supervisor.

6. Companionship & Emotional Support

- Provide respectful companionship and emotional support.

- Communicate patiently with elderly, bedridden, or dependent patients.

- Maintain a calm and positive environment.

- Respect the patient's privacy, dignity, preferences, and cultural needs.

7. Safety & Cleanliness

- Keep the patient's immediate surroundings clean, safe, and organized.

- Follow infection-control and hygiene practices.

- Take reasonable precautions to prevent falls and other household accidents.

- Use patient-care equipment responsibly.

8. Communication & Reporting

- Maintain clear communication with the patient's family and SVT supervisor.

- Immediately report changes in the patient's condition, falls, injuries, fever, breathing difficulties, unusual behavior, or other concerns.

- Report any absence, emergency, or difficulty in performing assigned duties promptly.

- Maintain appropriate records when required.

Professional Conduct

- Be punctual, responsible, polite, and respectful.

- Wear clean and appropriate clothing/uniform as instructed.

- Maintain patient confidentiality.

- Do not use the patient's belongings without permission.

- Do not argue with the patient or family members.

- Do not leave the patient unattended when supervision is required.

- Follow SVT Home Healthcare Services policies and instructions.

- Maintain professional boundaries with patients and their families.

Important

Caregivers must not independently perform medical procedures, administer prescription medication, change medical treatment, or make clinical decisions unless they are appropriately trained, authorized, and instructed by the responsible healthcare professional.

Emergency Responsibility

In an emergency:

1. Stay calm and ensure the patient is safe.

2. Inform the patient's family/responsible person immediately.

3. Contact the appropriate emergency medical service when necessary.

4. Inform the SVT Home Healthcare Services supervisor.

5. Follow the instructions of the healthcare professional.
